Additional Credit Card Documentation Required

•  shipment is to an international address
•  or the ship and bill addresses differ,
•  or the name on the card differs from the ship to name
•  or this is a military APO/FPO order,

•  or your credit card company can not verify your ship to address as being authorized by you as a valid ship to address. 
•  or the data you provided is inconsistent with your card issuers records or our requirements
•  or other as shown in our terms and conditions

So we may continue  processing your order , we need you to send your information as soon as possible and by one of the following methods

•  uploading the information or documents using the secure form below
•  or by faxing to our voice mail and fax handler* number (727-499-0945)
•  or send via ground mail* or other delivery means

Here is exactly what we need you to send:

1. a photocopy or scanned copy of your credit card
2. a photocopy or scanned copy of one of the following
   •  (a) Current Drivers License or
   •  (b) Current Passport or
   •  (c) Current Military ID Card

If you are an international customer:

•   and the price of any single item in your order exceeds $2700 (US$) you must also complete and submit this export form as required by the U.S. Government.
•   and you are paying by credit card, the order can only be shipped to the billing address of the credit card
